Must be a Connecticut resident.
Must be enrolled in Medicaid or eligible to apply.
Must need help with at least two or more Activities of Daily Living (ADLs) like bathing, dressing, eating, or mobility.
Must be eligible for services under the CT Home Care Program for Elders (CHCPE) or another relevant Medicaid waiver.
Must live with or move into the caregiver’s home, or have the caregiver move in with them.
Must be a Connecticut resident 18 Years or older.
Must pass a background check.
Must be physically and mentally able to provide.
Must be willing to complete orientation/training.
Must be willing to live in the same home as the person receiving care.
