whAT THE AFL PROGRAM IS.

What Is the Assisted Family Living Program?

 The AFL program is a state-supported Medicaid service that helps families care for elderly or disabled individuals in a home setting — with professional support and monthly caregiver compensation.

Rather than placing a loved one in a facility, the AFL program allows someone they trust (like you) to care for them at home — and get the resources to do it right.

Eligibility Criteria

Here’s who qualifies for the program:

For The Person Receiving Care:

  • Must be a Connecticut resident.

  • Must be enrolled in Medicaid or eligible to apply.

  • Must need help with at least two or more Activities of Daily Living (ADLs) like bathing, dressing, eating, or mobility.

  • Must be eligible for services under the CT Home Care Program for Elders (CHCPE) or another relevant Medicaid waiver.

  • Must live with or move into the caregiver’s home, or have the caregiver move in with them.

For The Caregiver:

  • Must be a Connecticut resident 18 Years or older.

  • Must pass a background check.

  • Must be physically and mentally able to provide.

  • Must be willing to complete orientation/training.

  • Must be willing to live in the same home as the person receiving care.

In many cases, caregivers are: Sons or daughters, Spouses (with restrictions), Siblings or extended family, Long-time friends or neighbors. If you're already providing care, you're likely closer to qualifying than you think.

Get Help With The Application

 We help you prepare the Medicaid application (if needed), CHCPE enrollment, and AFL caregiver documents.

Home Visit And Approval

 A case manager will assess the home and ensure safety and compliance with state guidelines.

Begin Care & Receive Payment

Once approved, you’ll be assigned a case manager, given caregiver support resources, and start receiving payment for your work.
